In-office initial consultation appointment required to receive your certification for a qualifying condition to use medical cannabis. An electronic written certification is required to complete the online patient registration and application for your medical cannabis card.

In-office or virtual appointment to complete required registration on the state portal website for you after your initial or recertification consultation appointment. We will complete all steps to ensure all you have to do is wait for your card to be issued.
